PRE-QUALIFICATION OF SUPPLIERS FOR 2027–2028
Banja La Mtsogolo (BLM), a Malawian non-governmental healthcare organisation, invites interested registered business entities to express their interest in consideration for the supply and provision of various goods and services for the years 2027 and 2028.
Location: BLM Support Office, opposite CFAO Malawi, along Paul Kagame Highway, Lilongwe.
Application Fee: MK40,000 per lot/category — non-refundable.

MANDATORY REQUIREMENTS
Applicants must provide:
- Fully completed Application/Questionnaire Form.
- Detailed company profile showing physical address, telephone contacts and valid email address.
- Authenticated bank statement for the last three months.
- Evidence of experience in the relevant category, including at least three recent LPOs/contracts.
- Valid Certificate of Registration/Incorporation.
- Valid VAT, TPIN and EFD certificates.
- Valid Tax Compliance Certificate.
- Certificate of registration with the relevant industry regulatory body, where applicable.
- Valid Public Procurement and Disposal of Assets (PPDA) Certificate.

The application form costs MK40,000 per lot/category, payable as a non-refundable fee.

Applicants should deposit the fee and present proof of payment when collecting the application form.
SUBMISSION OF DOCUMENTS
Completed application forms, together with the original bank deposit slip and all required supporting documentation, must be submitted in sealed envelopes.
The envelope should clearly indicate:
- Prequalification document name
- Reference code number
- Category description
Documents should be deposited in the designated BLM Tender Box at the Lilongwe offices, at the reception area, marked:
